A storyboard is a graphic representation of how your video will unfold, shot by shot. It's made up of a number of squares with illustrations or pictures representing each shot, with notes about what's going on in the scene and what's being said in the script during that shot.

Ann Flagella was a co-producer of the 2000 D&D film, and per her website is currently "a stand-up comedian's tour manager and talent agency."  Flagella is known bringing in Marlon Wayans who played Snails.  The other major actor in the movie was Jeremy Irons who portrayed the character, Mage Profion.

The film was released on December 8, 2000, so the book was shared before the actual release. Sweetpea is her production company, which is entirely female led. Sweetpea worked in conjunction with Behavior Worldwide & Silver Pictures. The movie had a budget of $45 million and was panned, bringing in a worldwide total just under $34 million.  I have been unable to find anyone named Jack or Jeffrey associated with the film’s production (it would be very unlikely to meet with something like this would go to an actor like Jeremy Irons), and based on the note indicating for perusal/use they may have been associated with the marketing of the film.

The note also mentions the binder houses the original storyboards while the spine indicates it holds the Dragon Sequence which is likely one chunk of the movie.   Using the numbers at the top of the page it  looks like its complete.
